Appointments

Welcome to our office
We look forward to meeting you. Our office is dedicated to helping patients with a jaw disorder, or with pain in the face, jaw, head, or neck. We are committed to supporting you in resolving your symptoms and concerns with the minimal treatment necessary.
To make an appointment, please call 801-328-3127. We are open M-Th from 7:00 am to 3:00 pm.
Prior to your appointment, we strongly suggest that you review all in the information on this site under the heading “Patient Education”. This will help you understand the nature of jaw problems. It will also prepare you to better understand the nature of your condition at the time of your consultation in our office.
Your initial appointment is necessary to gather records prior to your examination appointment. A Cat Scan of your jaws and jaw joints will be obtained. Vital signs will be taken. Your health and symptom questionnaire will be reviewed for completeness. The CT quality will also be verified. Following this visit, the CT will be read and analyzed by the doctor, and your symptom survey and health history will be reviewed.
Health Questionnaire and Symptom Survey
Your next appointment will consist of an interview with the doctor, a complete examination, and a consultation with Dr. Guinn. The nature of your jaw and other symptoms will be explained, and you will be given a treatment plan for the best way to resolve your concerns. We welcome your spouse or parents to be with you for this appointment, so they may also better understand your condition.
Please be aware that we do not call patients to remind them of their appointment. Because of the length of these appointments, we ask that you do not bring young children with you.
If you are currently using a bite guard or splint, please bring it with you to the examination appointment.
Insurance
Most insurance companies do not cover treatment for jaw disorders. Many have a specific exclusion clause that denies benefits for jaw problems. We cannot provide services contingent on insurance reimbursement. You will be directly responsible for all charges associated with your visits. Payment is due at the time of service. All major forms of payment are accepted. We will provide you with the forms to submit for insurance reimbursement.
