Please be aware that we do not call patients to remind them of their appointments. Because of the length of these appointments, we ask that you do not bring young children with you. Please allow extra travel time if you will be arriving before 9:00 am, as we are located on a busy street (Redwood Rd). You will find that our office does an excellent job running "on time". This can only be accomplished if YOU are on time for all of your appointments.
If you are currently using a bite guard or splint, please bring it with you to the examination appointment.
Most insurance companies do not cover treatment for jaw disorders. Many have a specific exclusion clause that denies benefits for jaw problems. We cannot provide services contingent on insurance reimbursement. You will be directly responsible for all charges associated with your visits. Payment is due at the time of service. All major forms of payment are accepted. We will provide you with the forms to submit for dental insurance reimbursement (Dr. Guinn is a dentist, so we are unable to provide you with medical claim forms).
